The way I would organise my lessons is by beginning with a short discussion. This would be about the topics that the student is struggling with. I would then find different questions based on that topic and ask them the approach that they would take. This would be to see where the student was going wrong in the question. Or I would look at their methods to see where they made a mistake.
After this, I would slowly go through the entire question from the beginning and explain the methods I am using and the reason I am doing it that specific way.
I would make sure that the student understood the way I was approaching the question. I would show them as I am doing it for example on a whiteboard, so they can see a step by step method.
After I finished going through the question, I would ask the student if they understood it all and if there was anything they wanted me to go over.
I would then pick a few other similar questions and let the student attempt those by themselves. I would check their answers and correct them.
Once they fully understood that type of question, I would then pick a harder type of question as part of the same topic and I would repeat this whole process again.
This is to ensure that they understand the whole topic as with maths, there can be many different styles of questions and many different approaches you could take.
